Submission of Applications

1.ÌýÌýÌýÌý Create a single PDF document that includes the following completed forms:

• Appropriate application forms (see options under "Types of General Academic Proposals")
• Supplement(s) to the application (as required)

2.ÌýÌýÌýÌý Create a separate PDF document for any required syllabi (see Task 3 for syllabi requirements.)

3.ÌýÌýÌýÌý Attach the PDF documents to an e-mail.

4.ÌýÌýÌý Send the e-mail toÌýocuerevadmin@nysed.govÌýwith theÌý³§³Ü²úÂá±ð³¦³Ù:ÌýInstitution Name, Degree Award, and Program Title.ÌýExample: AAA College, New Program, Master of Science, English Literature
